The protocols given here are very effective and have been developed, refined and tested in a busy modern clinic, catering to the needs of a variety of patients. In order to achieve good, and in many cases remarkably swift clinical results, they require the use of high dosages for several weeks. In patients who have poor digestion, it is advisable to begin the treatment below the recommended dosages, gradually increasing the dose, while making sure that the patient is able to tolerate the medicines without gastro-intestinal upset.
As with all protocols, those given here should be applied flexibly, with due consideration given to the aetiology and pathogenesis of each patient's individual condition. Thus, both the dosages as well as formula combinations should be prescribed appropriate to each patient's individual requirements.
Although each patient is unique and each disease state is also unique, there are some broad general guidelines that may be applied in our approach to designing a treatment regimen, specifically in regards to prognosis. The approximate course of treatment will vary according to the duration of the patient's condition:
- Chronic conditions: 1 – 3 years, or ongoing
- Sub-acute conditions: 3 – 6 months
- Acute conditions: 2 – 4 weeks
It is also important to bear in mind that once treatment of the acute stage symptoms has been successfully completed, it is often necessary to focus on the underlying imbalances that caused the condition. In this way, the treatment results can be consolidated, preventing recurrence of the condition. Thus, the second stage of treatment (for the underlying factors) may need to continue for 3 – 6 months or more depending on the age of the patient, the history of the illness and the patient's lifestyle.
The treatment protocols presented may vary as Traditional Chinese Herbal Medicine or Western Herbal Medicine. Eastern Medicine is advantageous as it incorporates syndrome diagnosis and standardised herbal formulas with a long track record of effectiveness and safety.
Clinical Protocols
In TCM, Acne lesions are related to the following patterns:
• Liver Fire
• Lung Heat
• Damp-Heat in the Liver-Gallbladder
Further development of these pathologies may lead to development of:
• Heat Toxin
• Blood stasis
• depletion of the Yin-Fluids
Therefore, when treating cases with acne it is best to begin by addressing the Liver Fire, and then the Lung Heat. In long standing or refractory cases, it is important to also consider activating the Blood to dispel stasis as well as replenishing the Yin-Fluids (using Si Miao Yong An Wan). Clinically, good results can also be achieved in stubborn cases with the addition of herbal combinations that have the action of promoting the Liver and Gallbladder and draining Damp-Heat (using Yin Chen Hao Wan – Jia Wei).

The main cause of angina (central chest pain) is Blood stasis in the chest. Therefore, the main principle of treatment is to activate the Blood and dispel stasis. Additional contributing factors are retained Phlegm and deficiency of the Heart Qi, which need to be addressed as secondary principles of treatment.
The main aim of TCM herbal therapy is to unblock the heart's blood vessels by removing obstructions, which may be achieved by using the formulas in the protocols below in a timely manner and at a high enough dosage. This is a better approach than that of Western medical treatment, which only acts to temporarily dilate the blood vessels.

Note: If the patient suffers frequent episodes of angina, an additional dose of Xue Fu Zhu Yu Tang (Persica & Cnidium Combination) may be taken at night. When there has been an improvement, return to the normal protocol. Xue Fu Zhu Yu Tang is a good treatment for older patients, as one develops increasing Blood stasis with age. This is particularly worsened in very cold weather (e.g. winter season) or very hot weather (i.e. summer season). In winter the cold causes the blood vessels to contract and in the summer, the heat causes the body to lose water and the blood becomes thicker. Therefore, angina patients may be given an additional dose of Xue Fu Zhu Yu Tang during these times as a preventative treatment. When the condition has improved, return to the basic protocols given above.
A very important, but often overlooked, feature of anxiety is the presence of turbid Phlegm ('misting or obstructing the clear orifices'), which clouds the mind. Most commonly the following TCM principles of treatment are utilised in the treatment of anxiety:
• Soothe the Liver and relieve constraint
• Nourish the Heart Blood
• Strengthen the Spleen and tonify the Qi

The key point in diagnosing and treating patients with asthma is to properly address the underlying deficiency patterns in each case (i.e. the ben factors). Generally the manifest symptoms (i.e. the biao factors) are readily apparent (i.e. Phlegm, Heat, Heat Toxin, Blood stasis). However if these factors alone are treated, clinical results will be poor with little change over the longer term.
The main deficiency patterns are Spleen and Kidney Yang deficiency, Lung Qi deficiency with deficiency of the Exterior and insecurity of the defensive Qi. At the deepest level, there is generally a combined deficiency of the Spleen Yang and the Kidney Yang. This is the root cause of the more apparent deficiency of the Lung Qi together with deficiency of the Exterior with insecurity of the defensive Qi.
In planning a treatment protocol, emphasis should be given to clearing away the biao factors, while consolidating the Exterior and strengthening the Lung Qi. Over the long term, treatment during the remission stage should continue for many months, with the aim of warming and tonifying the Spleen and the Kidney Yang. This is essential for most cases.

Note: Yu Ping Feng San (Jade Screen Formula) must be given in sufficient dosage to achieve a good clinical effect. If a patient does not respond well, this formula can be taken up to six times daily (at a dosage of 50 pills). However, if a patient does not improve with the above protocols that use Yu Ping Feng San, then Zhuang Yang Yi Jing Wan (Epimedum & Ginseng Formula) may be used instead.
The main pathodynamic in acute or acute on chronic bronchitis is Phlegm-Heat. Clinically, the treatment protocol should address Heat Toxin as well as the Phlegm-Heat. This is because Heat Toxin may readily develop as a consequence of the Phlegm-Heat, obstructing the Lung Qi.
During the remission stage, the treatment should focus on consolidating the Exterior and tonifying the Qi in children, and also tonifying the Kidney in middle aged and elderly patients
In order to make the treatment suitable for an individual case, the dosages of each formula may be adjusted up or down in accordance with the treatment principle that is being emphasized.

With the development of more effective treatments for various types of cancer by Western medicine, and the consequent improved survival rates for cancer patients, TCM clinics are more commonly being called on to help these patients who are experiencing the unwanted effects of treatment. In many cases TCM support can allow patients to continue with the Western treatment and thus complete the prescribed course.

In TCM, cases with elevated cholesterol or triglycerides (generally termed 'dyslipidemia'), are characterised by Phlegm and Blood stasis, together with Damp-Heat affecting the Liver and Gallbladder. Clinically, the good results are obtained by following the treatment principles: promote the Liver and Gallbladder, resolve Phlegm and activate the Blood to dispel Blood stasis. These protocols can effectively lower the abnormal lipids. Clinical progress should be monitored by monthly blood tests, rather than relying on signs and symptoms.

CFS is regarded as being caused by a chronic sub-acute viral infection. Over time, this leads to complex presentations with a variety of TCM patterns. This is because on the one hand the initial infection develops into Heat and subsequently Heat Toxin, while on the other hand the continued presence of unresolved pathogenic factors depletes the health Qi (zheng qi), resulting in a variety of different deficiency patterns.
Clinically, the main principles of treatment are to strengthen the Spleen and tonify the Qi; clear Heat- Toxin; and warm and tonify the Kidney Yang. Treatments that tonify the Qi and warm and tonify the Kidney Yang or tonify the Qi and enrich the Kidney essence are clinically very effective in long standing cases where the viral infection has burned itself out. These may also be applied to cases of simple fatigue, with relatively severe presentations.
Generally, these cases of CFS represent chronic conditions, which have developed over a long period. Therefore, they require a long-term course of treatment at sufficiently high dosage so that the treatment effects accumulate, leading to a good outcome.

In TCM common cold and influenza may be due to invasion by Wind-Cold or Wind Heat. In most cases Wind-Cold transforms rapidly into Wind-Heat. Clinically, the best results are obtained when Xiao Chai Hu Wan is combined with the main Exterior releasing formula in order to prevent deeper penetration of the pathogens and clear the Heat. Based on my clinical experience over several decades, Yin Qiao Wan is generally the better choice for releasing the Exterior in most cases.
The key to effective treatment of the common cold and 'flu is to use very high dosages of each formula (around 200 - 300 pills per day) and begin treating as early as possible, when the signs and symptoms are only mild. In this way it is possible to fully resolve it within 2 days and avoid developing a fully blown infection.
Please note that Protocol D is specifically for elderly patients or those with a weak constitution, in whom the infection has moved into the lungs.

Chronic constipation is generally due to constitutional type or advancing age of the patient. In the latter group the underlying cause is deficiency of the Qi and the body Fluids. In most cases the basic treatment formula is Run Chang Wan as it addresses the common pathodynamics, i.e. Dryness of the Intestines and stagnation of the Qi. In stubborn or long standing cases it is generally necessary to treat the Ben and Biao at the same time, using an additional formula to moisten Dryness and regenerate the body Fluids.

Although the basic pathodynamics in depression are related to impairment of the Heart, Liver and Spleen, consideration should also be given to the role of Phlegm. Resolving Phlegm is the key factor in the treatment approach. Other treatment principles include:
• Nourish the Heart and calm the Spirit
• Soothe the Liver to relieve constraint
• Activate the Blood and dispel stasis
• Nourish the Blood and the Yin
In menopausal patients with depression it is important to nourish and activate the Blood. This approach has the effect of tonifying the organs of the lower Jiao in order to regulate and balance a woman's physiology and function.

The following protocols apply to the treatment and management of early stage diabetes.
In the earlier TCM texts, diabetes, referred to as 'xiao ke', manifested with signs of Qi deficiency, Yin deficiency and Heat: severe thirst, constant hunger and polyuria. It was discussed according to upper, middle and lower Jiao symptoms.
However, in modern Western clinics, we do not see these types of patients, as they are generally detected and treated with Western medicine before they get to this stage. Therefore, the ancient theories regarding 'xiao ke' are not applicable in clinic today. Clinical experience shows Kidney Yang deficiency and Blood stasis are the main underlying pathodynamic factors. Therefore, the most important thing in treating all patients with type 2 diabetes is to tonify the Kidney. This is the essential principle of treatment for all different types and all different stages of the disease, including those cases with complications.
In cases of insulin resistant diabetes with elevated blood sugar levels and possibly also hypertension, the treatment principles of warming and tonifying the Kidney Yang, enriching the Kidney Essence, activating the Blood to dispel stasis will generally have good clinical results.
It is important to note that treatment should continue over the long term, using the doses specified below.

The essential pathodynamics in emphysema are a combination of Qi deficiency, Spleen and Kidney Yang deficiency and Blood stasis in the chest.

The characteristic clinical features of eczema arise when Wind together with Damp and Heat attack the skin. Cases with fluid filled vesicles and/or exudation are due to the additional presence of Heat Toxin in the skin. As Wind is responsible for the severe itching experienced by patients the most important treatment principle is to expel Wind using Xiao Feng San, which is the main formula in treating this condition. Additionally, most cases require treatment to clear and drain Damp-Heat, using Long Dan Xie Gan Tang.

Fatigue is a common clinical presentation and care must be taken to distinguish cases of CFS from those with simple fatigue. If there is a recent history of viral infection, CFS is more likely. If there are no signs of infection or presence of pathogenic factors, the patient has simple fatigue.
In order to obtain rapid clinical results, strong tonification therapy is required. The best approach is to begin by tonifying the Spleen, the Kidney and the Yang Qi all at the same time. Once there has been a marked improvement in the patient's condition, the treatment should continue in a simplified form, appropriate to the patient's needs (i.e. select one or two of the formulas given below) and continue for several months at a lower dosage.

Hypertension has a complex pathogenesis, involving Blood stasis, Liver Wind, turbid Phlegm, and Liver- Kidney Yin deficiency. The primary treatment principle is to address the Blood stasis and therefore the most appropriate base formula is Xue Fu Zhu Yu Tang. By using this formula as the basis for treatment, other formulas may be added, depending on the chosen principle of treatment, e.g. to calm the Liver Yang, resolve Phlegm or nourish the Liver-Kidney Yin. Therefore, it is important to pay attention to the clinical features of each individual patient in order to prescribe the suitable formulas according to TCM differential diagnosis.
Generally, a prolonged course of treatment is required for the cumulative effects of the prescribed formulas to produce a good clinical outcome. In addition, the formula Xue Fu Zhu Yu Tang is highly effective when taken by elderly patients in maintaining the health of the blood vessels, and has great value in prevention of stroke and heart attack.

The key pathodynamic feature of this disorder is Kidney deficiency together with Blood stasis. Patients may present either with Kidney Yang deficiency, or Spleen and Kidney Yang deficiency, together with Kidney Essence deficiency. In addition, Blood stasis is the other important underlying pattern. Thus, the most common treatment principles include: warm-tonify the Spleen and Kidney Yang; benefit the Kidney and replenish the Essence, activate the Blood and dispel stasis.
In treating patients with both deficiency and Blood stasis, an alternating treatment protocol is often effective. The patient is prescribed either protocol A or B for one week, followed by protocol C for the next week. The patient should be reassessed every month and treatment continued with the appropriate combination of formulas.
As this is a long term, stubborn condition, it is important to give long term treatment, using high doses, as specified below. This is not a modern disease, as the clinical features have been described since ancient times in TCM literature. By taking the traditional approach and treating the root causes (i.e. Kidney Yang and Essence deficiency) over a sufficient period of time, this condition can be resolved.

In approaching women with infertility it is important to bear in mind that in addition to the various causes of this problem, consideration should also be given to the patient's body constitution. Generally, there are five major causes:
1. Ovulation disorder – irregular or intermittent ovulation: treatment is to promote ovulation and use herbal formulas to control the cycle.
2. Irregular menstrual cycle, generally due to failure of the corpus luteum to produce sufficient progesterone. Treatment is to use herbal formulas to control the cycle

There are three main pathodynamic factors underlying insomnia:
• Liver Qi constraint, Qi stagnation (i.e. emotion - related stagnation of the Qi)
• Phlegm
• Blood stasis
It is important to differentiate these patterns in clinic and apply the appropriate treatment. It should be noted that the main protocols rely primarily on one or more of the above treatment approaches without using a specific Spirit calming (an shen) formula. The common underlying patho-mechanism begins with emotional strain, which affects the Liver. The Liver then begins to fail in maintaining the patency of the Qi, leading to Qi stagnation. The two main consequences of this are Blood stasis and Phlegm, which may lead directly to sleep disturbance. Thus, the basic treatment principles in insomnia center on activating the Blood to dispel stasis, resolving Phlegm and moving or regulating the Qi.
In older patients, who tend to develop Blood stasis, the formula of choice is Tao Hong Si Wu Wan, which, in my experience, is generally more effective than Xue Fu Zhu Yu Tang.
In menopausal women, the common underlying factors are Blood stasis and Phlegm. These are also common underlying factors in depressed patients who are unable to sleep.
Finally, the approach to treatment should be based on the understanding that patients with different body constitutions will require different treatments; while patients with the same body constitution (or even the same patient who presents at a different time) may also require different treatments depending on the various stresses and lifestyle factors involved. Therefore, a flexible approach is required, basing each patient's treatment on a careful assessment of signs and symptoms and TCM pattern. In cases where satisfactory results have not been achieved with Protocols A and B, an additional formula may be select according to the presenting TCM pattern, to be used in combination.

There are two basic pathodynamic factors that together underlie most cases of IBS: a) Liver constraint, Qi stagnation, together with b) Spleen Qi deficiency. These give rise to the characteristic clinical features of abdominal pain and distention, diarrhoea or constipation.
Although individual patients may vary in their presenting symptoms and signs, and one pathodynamic factor may appear to predominate, it is important to treat both, ensuring that the treatment principles include soothing the Liver to relieve constraint, and also strengthening the Spleen to dry Damp.
It should be noted that cases with abdominal cramping pain but no alteration in bowel function (i.e. not true IBS) should be treated with Shao Yao Gan Cao Tang together with Chai Hu Shu Gan Wan to soften the Liver and resolve Qi stagnation.
In cases with severe diarrhoea, treatment should focus on strengthening the Spleen, warm-tonifying the Spleen Yang, drying Damp and astringing the Intestines.
The root causes (ben) of this condition are due to Kidney Yang deficiency, manifesting in an auto-immune response or inappropriate immune response. Protocol D addresses this, and by tonifying the Kidney Yang, assists in regulating and harmonising the immune functions.

Generally, when the Kidney Yang is depleted the Kidney Yin also requires to be nourished. This is especially important in older patients.
In the treatment of cases that present with signs of Blood stasis (i.e. body aches, headaches, purple patches on the tongue), it is important to tonify the Spleen Qi and the Kidney Essence, in addition to activating the Blood to dispel stasis.
Surprisingly good results are obtained by advising male patients to take Zhuang Yang Yi Jing Wan (to warm-tonify the Kidney Yang and replenish the Kidney Essence) about 20 – 30 minutes before intercourse. There are three clinical scenarios in which an approach that is similar to that used by Western naturopaths, referred to as a liver detox, may be used.

The distressing physical symptoms of menopause, i.e. hot flushes, are due to deficiency Heat; while the symptoms of dryness are due to Yin deficiency. Menopausal syndrome may also manifest with emotional changes such as insomnia, poor memory and concentration, mood swings, etc., as well as loss of general vitality and body strength.
The treatment approach recommended is to treat the underlying causes (ben) by tonifying the Kidney (both the Kidney Yin and the Kidney Yang), nourishing the Yin-fluids and resolving Phlegm. The protocols presented here are no less effective than HRT, without the risk of unwanted adverse effects, such as breast cancer. This formula is suitable for all the different clinical presentations, and in most cases it is used in combination with formulas to nourish the Kidney Yin and Essence, nourish the Blood, and activate the Blood to dispel stasis.
Although in some cases, the application of a single formula from Protocol A may be effective in the relief of hot flushes and sweating, more lasting results are achieved with the combinations of formulas, as listed in the various protocols below.
In menopausal women who present mainly with mental-emotional symptoms, the additional treatment principles required are to resolve Phlegm and also to stabilise the mind and emotions. Psoriasis is the most difficult to treat of all skin diseases. It is a stubborn condition that tends to recur. It belongs to the category of auto-immune disorders. The treatment approach is to address the main underlying pathodynamics:

Rheumatoid arthritis is a type of auto-immune disorder in which the joints are attacked and become inflamed and deformed. In TCM, it is classified as 'stubborn Bi', because it is often refractory to treatment. It is a type of Bi syndrome due to Wind and Damp, and often also Heat Toxin. The main clinical features are redness, pain and deformity of the affected joint/s.

The spurge family (Euphorbiaceae) contains around 7500 plant species, with several traditionally used for medicinal purposes. Euphorbia peplus, commonly known as petty spurge in the U.K. or radium weed in Australia, has a long history of use for a variety of conditions. The sap from this plant has been used as a purgative and as a treatment for warts, corns, waxy growths, asthma, catarrh, skin cancers, and cancers of the stomach, liver and uterus.
Cases of home treatment of basal cell carcinoma have been reported. In a survey of home remedies for skin cancer and solar keratoses, topical treatment with the sap was unanimously considered to be effective by the users. There is a long history of community use with the absence of documented side-effects, except in cases of accidental ocular exposure.
Protocol
Treat topically with the sap of Euphorbia peplus once daily for three consecutive days by using a cotton bud to apply sufficient sap to cover the surface of each lesion. Take care to prevent the liquid dispersing to the adjacent skin. Use a transparent waterproof dressing to cover the skin legion between applications.
Treatment Site Reactions
The post-treatment local acute skin toxicity was zero in 6%, erythema in 6%, dry skin desquamation in 51%, patchy moist desquamation in 28% and necrosis in 9%. Skin reactions returned to normal within 1 month for 62% of the lesions and thereafter only mild erythema persisted for an average of 4 months for 38% of the lesions. No pain was reported by 43% of patients, 37% reported mild pain, 14% reported moderate pain and severe pain (requiring analgesics) was reported by one patient (6%). When encountered, pain was always localised to the site and lasted from 2 hours to 2 days. In all cases of successful treatment, a favourable cosmesis was observed.

When treating patients with nervous tension or stress, the main clinical focus should be to resolve Phlegm, using Wen Dan Tang. The main cause of urinary tract infection is Damp-Heat affecting the lower Jiao (i.e. Bladder and urinary tract). Thus the primary treatment principle is to clear Damp-Heat and Heat Toxin from the lower Jiao, reinforcing the treatment with the 'magic'* formula Si Miao Yong An Wan, which clears Heat Toxin, activates the Blood to dispel stasis and nourishes the Yin. This formula provides remarkable efficacy in such conditions and it is regarded as the key treatment formula. Never-the -less, it is very important to follow the dosage schedule outlined below, as smaller dosages will not provide effective clinical results.
